Registration of Death

Information about the death of a Sri Lankan citizen must be notified to the Embassy by a relative, friend or any other known person who was an eyewitness to the death.
Required documents for the registration :
1. Death report by the Doctor with an English translation
2. Certificate issued by the hospital which hand over the body with an English translation
3. Other related documents of the death issued by the Japanese authorities with English translations
4. The Passport, Birth Certificate and/or other identification documents of the dead person.
5. Fee: JPY 6,500
