In lieu of passports which have run out of pages or expired

1.    Duly filled application form- Form K (IM35),
2.    4 colour photographs (3.5.x4.5 cm)
3.    Original birth certificate
4.    National Identity card
5.    Marriage certificate (if changing the maiden name)
6.    Academic/Professional Certificate or letter of appointment (if the job description is to be included)
7.    Previous Travel Document
8.    Citizenship Certificate if the applicant is a citizen by registration
9.    Two self addressed envelopes

Charges for a new passport (PPT):

(i) If submitted application for the first time to obtain a new passport
Fee: JPY 16,000 + Postage JPY 800 = Total JPY 16,800
(ii) If produced M and N series of his/her passports, along with the application
Fee: JPY 16,000 + Postage JPY 800 = Total JPY 16,800
(iii) If produced J, K and L series of his/her previous passports, along with the application
Fee: JPY 34,000 + Postage JPY 800 = Total JPY 34,800

Note:-
The applicant should submit originals with two sets of photocopies for item Nos. 3 to 8.
